AveoMedical is a new patient-centered care platform for senior living. We deliver mobilized primary care services that are preventive and technology enabled through telehealth and an advanced patient portal to bring a higher level of service and communication to patients, providers, caregivers, and families. Our providers have a better work life balance and can actually build lasting relationships with their patients for healthier lives. We strongly believe that the US healthcare system is broken and we need to start by changing the way it is delivered, from a volume-based system to a value-based system. We are seeking the following roles to join the AveoMedical team: Primary Care Physician, Mid-level Provider, Patient Care Coordinator, Telemed Night Nurse, and Clinic Admin Assistant.

JOB POSTING #1

Primary Care Physician

DALLAS OR FORT WORTH

SPECIALTY & EXPERIENCE:

Internal Medicine or Family Medicine

Internist with geriatric fellowship preferred, but not required

If Family Medicine, extensive experience with geriatric population required

Time requirements:

Medical directorship position, with part-time primary-care work to start that will ramp up into a full-time position. Professional services are a mix of onsite and virtual telemedicine visits.

Responsibilities:

  1. The ideal candidate will admit, evaluate, diagnose, treat and provide consultation to patients (i.e. members) 50 years of age and older with common and complex illnesses, afflictions, disease, and functional disorders of the circulatory, respiratory, digestive, endocrine, metabolic, musculoskeletal, hematopoietic, and eliminative systems of the human body.

  2. The candidate should be a team player and display authentic compassion given the nature of the geriatric demographic. Enjoys working closely with mid-levels, RNs, MAs, care coordinators, health coaches, home health providers, and communicates well with patients and colleagues.

  3. Possess leadership skills to manage team of mid-levels, coordinate care plans and sign off charts in a timely manner. Display a high level of proactive communication to address chart findings and treatment actions that are not in line with physician orders. Manage professional relationships, gain rapport, and lead care teams to execute care plans.  

  4. Availability throughout the week required to take calls, emails, texts, as well as telemedicine and in-person visits with members or member’s families and caregivers. Mid-level and care coordinator support to compliment care plan execution.  

  5. Technologically savvy to use EHR, care coordination, and patient portal technologies that consume wearable data to take action on insights through data and trigger best of breed medical practices and wellness programs.

CompensatioN:

Competitive monthly stipend for medical directorship

Competitive hourly rate for all in-person and virtual visits

Potential for profit interest share in local market

Licensure or Certification:

Board Eligible or Board Certified (BE/BC) in Internal Medicine or Family Medicine

Fellowship in Geriatrics or Integrative Medicine preferred

Functional Medicine certification also a plus

Nurse Practitioner or Physician Assistant

Integrative and or geriatric specialties preferred

Time requirements:

Part-time position to start, with opportunity to ramp up into full time. Visits are a mix of onsite and virtual telemedicine.

Responsibilities:

  1. Take comprehensive histories and provide physical examinations and other health assessment and screening activities.

  2. Diagnose, treat, and manage patients with acute and chronic illnesses and disease.

  3. Order, perform, supervise, and interpret laboratory and imaging studies.

  4. Prescribe medications, durable medical equipment, and advanced therapeutic interventions.

  5. Develop specialized psychomotor skills in the performance of procedures.

  6. Initiate health promotion, disease prevention, health education, and counseling.

  7. Collaborate and communicate with members of the inter/intra professional healthcare team.

  8. Assess, educate, and provide referrals for the patient, family, and caregiver.

  9. Seamlessly implement transitions in levels of care.

JOB POSTING #3

Patient Care Coordinator

DALLAS OR FORT WORTH

SPECIALTY & Experience:

RN, LVN, or Medical Assistant

Care Coordination and/or Social Worker backgrounds are preferred

Time requirements:

Part-time position to start, with opportunity to ramp up into full time. Visits are a mix of onsite and virtual telemedicine.

Responsibilities:

  1. Serve patients by greeting and assisting them with utmost customer service; triage phone calls, schedule appointments, verify benefit eligibility, maintain records and accounts in all DFW area locations.

  2. Orient and educate patients and their families by meeting with them, explaining the role of the patient care coordinator, initiating the care plan, and providing educational information in conjunction with direct care providers related to treatments, procedures, medications, and continuing care requirements.

  3. Obtain medical records when requested by a provider.

  4. Develop interdisciplinary care plan and other case management tools by participating in meetings, coordinate information and care requirements with other care providers, resolve issues that could affect smooth care progression, foster peer support, and provide education to others regarding the case management process.

  5. Monitor delivery of care by completing patient rounds, document care, identify progress toward desired care outcomes, intervene to overcome deviations in the expected plan of care, and review the care plan with patients in conjunction with the direct care providers.

  6. Interact with involved departments to negotiate and expedite scheduling and completion of tests, procedures, and consults; report personnel and performance issues to the unit manager, maintain ongoing communication with utilization review staff regarding variances from the care plan or transfer/discharge plan.

  7. Evaluate outcomes of care with the interdisciplinary team by measuring intervention effectiveness with the team; implement team recommendations.

  8. Respect patients by recognizing their rights; maintain confidentiality.

  9. Maintain quality service by establishing and enforcing AveoMedical’s organization standards.

  10. Take vitals and maintain patient care database by entering new information as it becomes available; verify findings and reports, and back up data.

  11. Review and analyze patient data reports (e.g., discharges, hospital admissions, read missions, skilled nursing facilities). Follow up as necessary.

JOB POSTING #4

TELEMED NIGHT NURSE

TELEHEALTH / REMOTE

SPECIALTY & EXPERIENCE:

RN

Telephone/Telemed triage experience preferred

TIME REQUIREMENTS:

Part-time position for overnight shift work. One shift or broken down into two shifts, 7 days per week, with flexibility to work only weekends, only weekdays, or both. Visits are 100% virtual via telemedicine.

RESPONSIBILITIES:

  1. The Night Nurse provides clinical telephone and video conferencing support to patients within senior living communities (independent and assisted living environments). Night Nurse calls will involve assessment of patient’s acute care needs and appropriate case disposition to ensure positive patient outcomes.

  2. Includes coordination with onsite senior living community staff (i.e. med aides), hospital based providers, and other members of the patient’s care team to ensure execution of medical decision and appropriate documentation.

  3. Prioritize the order of callback to patients based upon their reported clinical problem(s), severity of injury, and service level agreements in place with partner community.

  4. Conduct sound nursing assessments and assist callers with issues related to patient care utilizing active listening skills in order to receive detailed information in an accurate manner to decipher diagnosis if ER admission is required or not.

  5. Prompt scheduling of onsite follow up provider visits with attributed PCP team.

  6. Possess strong organization skills to document and record Patient Triage Encounter Reports and share with approved staff, providers, and family members on record within a timely manner.

  7. Demonstrated ability to maintain composure when confronted by difficult situations and to respond professionally.

  8. Possess strong customer service skills to promote pleasant and effective interactions with patients, community staff, and providers.

  9. Perform designated nursing procedures within the scope of training and conduct follow up communication in accordance with diagnoses, policies, and procedures.

  10. Check for laboratory results, diagnostic-testing results, x-rays, call for results when necessary.

  11. Adhere to organizational policies, procedures and regulatory compliance requirements and directives.

RN; BSN degree preferred

CLINIC ADMINISTRATIVE ASSISTANT

DALLAS

SPECIALTY & EXPERIENCE:

Experience with or knowledge of Medicare preferred

Geriatric or Senior/Adult disabilities experience preferred

~1 year of healthcare work experience preferred

TIME REQUIREMENTS:

Part-time position with flexible working hours. Preferred working hours 9AM to 12PM, three to five days per week, Monday to Friday.

RESPONSIBILITIES:

  1. Primary duties involve administering daily clinic activities and supporting the clinical team under supervision of Patient Care Coordinator.

  2. Cheerfully meet and greet patients, serving as the first point of contact.

  3. Professionally answer phone calls in a timely manner.

  4. Schedule (New, Existing, Follow-Ups, Special Examinations, Walk-ins) patient appointments.

  5. Proficiently build and analyze clinical reports from EHR for patient outreach to schedule follow ups for preventive services like Annual Wellness Visits, Chronic Care Management, Care Plan Oversight, Diabetes management, etc.

  6. Work with patients’ insurance companies to ensure coverage. Collect co-payments or deductibles, demographic information, signed consent forms, and insurance information.

  7. Scan, file, and organize patient documents in EHR with attention to detail to ensure complete and accurate information.

  8. Willingness to actively learn and be intellectually curious.

  9. Track record of being ethical, reliable, emphatic, and detailed oriented.

  10. Exhibits high character and strong work ethic.

  11. Functions well in a team and is coachable.

  12. Proficient in MS Office, with emphasis on MS Excel.

LICENSURE OR CERTIFICATION:

Minimum of high school diploma or general education degree (GED)

Must have at least one year of medical office or clerical experience

Prefer patient registration experience

HIPAA certification preferred
